Washington County FC - Formal Registration Forms and Process

Formal registration is not done until after tryouts and players have been offered positions on teams.  That section is below.

Pre-registration is done prior to tryouts.  Pre-registration includes completion of an on-line form, and a simple printed form.  The on-line form must be completed at least one day before your tryout.  The printed form must be taken to your tryout.  See the Tryouts page for details.

INSTRUCTIONS:

In order to expedite the registration process and answer any questions you may have, we will be holding registration open houses on ___TDB___ at ___TBD___ (rain or shine).  Please bring completed forms to one of these open houses.  If you can't make either night, please mail all forms and fees to WCFC P.O. Box 773, West Bend WI 53095.

This year everyone will be ordering new uniforms. 
We will have samples at registration night for players to try on. 
Including 2 Jersey’s, 1 pair of shorts and 1 pair of socks.
Please note that uniform costs are in addition to player fees.

Print a copy of each form provided below, fill them out completely, collect the other items (including photo!) on the checklist, and mail them or turn them in at a registration meeting.  Please fill in all fields and the required signatures.

WCFC Required Registration Forms and Items Checklist

1) WYSA Player Registration Form

2) Liability Release & Medical Consent 2010/2011 Form.

3) WCFC Player Code of Conduct agreement

4) WCFC Parent Code of Conduct agreement

6) WCFC Uniform Order Form - Everyone will get new uniforms this year

7) $85 check made out to WCFC for uniform - This year everyone will be ordering new uniforms.  We will have samples at registration night for players to try on.  Including 2 Jersey’s, 1 pair of shorts and 1 pair of socks.

8) Copy of a Certified Birth Certificate if the player is new to the club.

9) 1" x 1" photo of the player (head). 

10) Player's signature on a small white piece of paper.

11) Check for player fees made out to WCFC.  U11-U14 $400.00.  U15+ $350.00.  Please deduct $35.00 if you paid the try out fee already.  Please note that these fees do not include a new Uniform.  Only new players are required to purchase new uniforms this year ($85 - see 6)&7) above).
